Post Rock Family Medicine is composed of five separate practices: Daniel J. Sanchez, MD, Prairie Star Family Practice, LifeLine Family Medicine, Rooks County Health Center Clinic, and Solomon Valley Family Medicine. We have clinics in Plainville, Stockton and Palco.

We all work together as a team to coordinate your care; when you see any of PRFM’s providers, that information is shared back to your primary provider through the electronic health record.

Our office hours are 8:30am to 5:30pm, Monday through Friday.  You can also find our providers staffing Doctors Without Delay, a walk in clinic at Rooks County Health Center, open 8:00am to 6:00pm, Monday through Saturday.

Post Rock Begins Using Televox Electronic Reminders

Post Rock has begun using Televox, an automated telephone reminder system this week.   This new system will electronically contact patients between 6pm and 8pm the night prior to a scheduled visit with any of the Post Rock medical providers.  Patients or household members answering the phone will only hear the patient’s first name and that an appointment has been scheduled.  No private information will be disclosed through these calls.

If you have caller ID, then you will see our primary contact number when the call comes through, (785) 434-2622.

Post Rock is continuing to look into other potential opportunities with this system.  Televox may eventually be set up to alert you through text or email.  Further, the system will eventually be used to remind patients of billing updates and when lab work is due.

Most people set a new year resolution to “get healthier.”  For some people this means eating better, for others spending time on the treadmill, for most it means losing weight.  Post Rock would like to help our patients achieve their goals in 2012.

PRFM is offering participation in the Virtual Lifestyle Management (VLM) project to our patients who have a body mass index (BMI) of 30 or greater.   VLM is designed to help people learn about healthier behaviors and lose weight. You can determine if you qualify for the project by calculating your BMI (click here).

Post Rock Reaches Out to Read!

Getting books from the doctor is now a routine part of regular well child checkups at Post Rock Family Medicine, as doctors and nurses welcome the Reach Out and Read program to the practice.  Post Rock joins more than 4,688 Programs nationwide that are preparing America’s youngest children to succeed in school.

The program begins at the 6-month checkup and continues through age 5, with a special emphasis on children growing up in low-income communities. Families served by Reach Out and Read read together more often, and their children enter kindergarten with larger vocabularies and stronger language skills, better prepared to achieve their potential.

“We are very excited to have introduced Reach Out and Read at Post Rock this year!  Giving books to children at well checks is a lot of fun, and I get to share some of my favorite childhood books with a whole new generation,” says Dr. Dan Sanchez.
